BEACHSIDE ACTIVITIES CLUB LIMITED

I have enrolled myself or the below listed participant(s) (child or children) in a programme or service of Beachside Activities Club Limited. I understand the participant’s participation in the programmes and services involves exposure to inherent activities including, but not limited to, surfing, body boarding, kayaking, swimming, running, paddling, team sports, and other beach related sports and activities, including all necessary travel and transportation to and from Beachside Activities Club Limited. I understand the inherent risks in the activities offered by Beachside Activities Club Limited cannot be eliminated. Individually, as a participant, and as the parent or guardian of the participant, I hereby expressly assume all risks associated with mine or the participant’s participation in Beachside Activities Club Limited including all risks associated with ocean water sports, beach activities, and necessary travel, and that I am aware that I myself or the participants will use equipment intended to improve or enhance the mine or the participant’s skills.

Despite my understanding of the forgoing risks, I, individually and as the participant or parent or legal guardian of the participant, FOREVER AGREE NOT TO SUE AND TO RELEASE FROM LIABILITY AND TO DEFEND, INDEMNIFY AND HOLD HARMLESS Beachside Activities Club Limited, owners, sponsors and affiliates, employees and agents, and volunteers for any and all damage or injury, known or unknown, arising out of mine or the participant’s participation in Beachside Activities Club Limited regardless of the cause, including negligence. I further authorize any physician, surgeon, paramedic, lifeguard, or nurse to render medical care when necessary, the costs of which are my sole responsibility. I understand that the forgoing is a LIABILITY RELEASE and a MEDICAL AUTHORIZATION that is now legally binding on me, the participant, our heirs and our legal representatives and I sign it off on my free will.

I agree to accept full responsibility, financial or otherwise, for my child’s actions. If my child is dismissed from the club for improper conduct, I understand that Beachside Activities Club Limited will not refund registration or enrollment fees. I agree to accept full responsibility, financial or otherwise, for all actions/requests by me via telephone, mail, fax, email, Internet, or any other form resulting in any charges/fees associated with such requests. I authorize Beachside Activities Club Limited to take photographs of my child or children during club activities and use these photographs on promotional and/or informational materials, such as posters, websites, emails, etc.

Payment policy
The registration fee of $15 plus a full payment is required to confirm participant(s) enrollment.

Refund policy
There are no refunds or credits for any reason.

Absences and make up days
There are no refunds given for days missed. Subject to availability, each participant will be allowed two (2) make up days. If your child misses a day of the programme, you will need to schedule a make up day by calling the office. You are required to schedule make up days and may not show up at the site for a make up day unless it has been previously scheduled.
